Mission Statement

No matter what shape your addiction takes, there’s no shame in admitting to having a problem, and there’s no shame in acknowledging that we all need help sometimes. At KAV’s House, we aim to help our residents regain control of their lives and learn to live beyond their addictions by offering peer support, case management, and a welcoming community. Our recovery housing is located throughout Ohio.

Vision Statement

Transforming lives through compassionate care, our vision is to provide a safe, structured, and affordable home for individuals overcoming substance use disorder. In this nurturing environment—free from harmful substances—they find hope, support, and the opportunity to reimagine their future.

Lease / Resident Agreement / Rent

A lease agreement (or lease) is a written document that establishes or modifies the terms, conditions, rules, and other provisions related to the use and occupancy of residential premises by one of the parties. All residents are required to sign a lease agreement before moving into KAV’s housing.

Lease agreements at KAV’s are structured on a week-to-week basis for both the lessee (resident) and the lessor (KAV). KAV reserves the right to renew or not renew the lease on a weekly basis. Additionally, KAV may increase the rent with a 30-day written notice.

Resident Agreement Will Cover the Following Areas

  1. Fees: Including the application fee (which covers urine screenings).
  2. Week-to-Week Lease: All residents will be on a week-to-week lease agreement.
  3. Lease Termination: Clear guidelines on how both the lessor (KAV) and the lessee (resident) can end the lease.
  4. Sobriety Requirement: Izzy’s is a sober living home. No drugs or alcohol are allowed on the property, and residents must provide negative urine screens to remain in compliance.
  5. No Refund Policy: All fees and rent payments are non-refundable.
  6. Abandoned Property: Explanation of what happens to a resident’s belongings if they leave the property—whether with or without notice—and do not arrange for retrieval.

The Fair Housing Act (FHA) prohibits discrimination in the rental (including services), sale, advertising, design, insurance, and financing of dwellings, as well as in other housing-related transactions, based on membership in protected classes. These protected classes include race, color, familial status, religion, sex/gender, national origin, and disability. Additionally, Ohio state law extends protections to include ancestry and military status.

Rent Collection Policy

Due Date & Payment Method

Rent is due every Friday by 2:00 PM and covers the upcoming week. Timely payment is required to remain in good standing. Rent must be paid by debit or credit card only. Residents can make payments directly to the Operations Manager or by calling the office.

Receipts & Recordkeeping

Once payment is received, a receipt will be sent via text message and/or email. Residents may also request a printed copy from the office. KAV tracks rent using both a digital spreadsheet and a physical receipt book. Each resident will receive a receipt for every payment.

Lease Terms

Residents sign a week-to-week lease upon move-in. A new lease is not required each week; the lease automatically carries over. If any changes are made to the lease terms, KAV will provide a 30-day written notice, and a new lease will be signed.

Late Fees

If rent is more than one week late, KAV reserves the right to apply a 5% late fee based on the total rent due.

Eviction Policy

Residents may be evicted for:

  • Non-payment or repeated late payment of rent
  • Violations of house rules (e.g., not attending treatment, theft, refusal to complete drug screens)

If a resident falls more than one month behind on rent, the following steps will be taken:

  1. The resident must meet with the Home Manager to create a payment plan.
  2. If the resident fails to pay rent within two weeks of the plan being established, they will be asked to leave the residence.

Community Standards & Accountability

KAV is a community living environment focused on supporting residents in their recovery journey. While we strive to work collaboratively with each resident, maintaining a safe, sober, and respectful environment is essential. KAV is not responsible for any items that are stolen, broken, lost, or damaged.

Client Personal Items Policy

To ensure a clean, organized, and safe living environment, residents are expected to limit personal belongings according to the following guidelines.

Clothing & Footwear

  • Up to 14 tops and 14 bottoms (includes pants, shorts, dresses, hoodies, sweaters, etc.)
  • 10 undergarments (underwear, bras, undershirts, etc.)
  • 7 sets of pajamas
  • Up to 5 pairs of shoes

Electronics & Entertainment

  • One TV, no larger than 32 inches
  • Radio or alarm clock
  • Game system, allowed only for use with personal TV
  • One laptop and/or tablet

Hygiene & Personal Care

  • Personal hygiene items limited to 10 bottles per person (e.g., shampoo, conditioner, lotion, body spray, gel, hairspray, shaving cream, etc.)
  • Hair styling tools limited to 3 per person (e.g., hairdryer, curling iron, straightener)

Linens & Bedding

  • Up to 2 sets of sheets, pillows, and comforters
  • No furniture is allowed
  • No more than 3 stuffed animals

Decor & Personal Items

  • No more than 5 personal items displayed on your dresser or nightstand (items must be neat and organized; includes photos, plants, etc.)
  • Up to 10 books per resident

Appliances & Electrical

  • One fan per person
  • One extension cord or one power strip per person

Bags & Storage

Maximum of:

  • 2 backpacks
  • 3 purses
  • 2 duffel bags

Prohibited Items

  • Pets
  • Mini fridges
  • Wax warmers
  • Candles or any items considered a fire hazard

Vehicles

Residents may keep a car on the property; however:

  • All parking rules for each house must be followed
  • Vehicles must be in working condition

If a resident leaves sober living, management reserves the right to tow the vehicle after 48 hours

Important Notice:
KAV is not responsible for personal belongings that are lost, stolen, damaged, or broken. Renters insurance is strongly encouraged.

Resident’s Rights

When residents enter KAV/Izzy’s sober living, they will be asked to sign a copy of the Resident’s Rights. These rights will also be posted in a common area of each house. The signed copy will be kept in the resident’s file.

Residents have the right to:

  • Be verbally informed of all resident rights in a manner they can understand.
  • Request a written copy of all resident rights and the grievance procedure.
  • Exercise their rights without reprisal, except when such exercise conflicts with health and safety considerations.
  • File a grievance in accordance with the house policy.
  • Be treated with courtesy, respect, and consideration for personal dignity, autonomy, and privacy.
  • Confidentiality of communications and personal identifying information, within the limits required by state and federal laws and regulations.
  • Access their own records.
  • Not be discriminated against based on race, ethnicity, age, religion, gender, national origin, sexual orientation, physical or mental disability, developmental disability, genetic information, HIV status, or any other category protected by local, state, or federal law.
  • Practice a religion of their choice or abstain from religious practice.
  • Be informed in writing of the rates charged by the recovery house and any additional fees.
  • Not be locked out of the recovery house at any time.
  • Not be locked in the recovery house at any time for any reason.
  • Consult with an independent treatment specialist or legal counsel at their own expense.
  • Privately meet with staff from the Ohio Department of Mental Health and Addiction Services.
  • Not be deprived of any legal rights, including landlord-tenant and fair housing rights, solely due to residency in the recovery house.
  • Retain personal property and possessions, unless restricted by house policy.
  • Receive a full explanation if housing privileges are lost or restricted, including the methods for reinstating those privileges.
  • Request and receive, in a timely manner, a written receipt for any payments made or a statement detailing any expenses, charges, and payments.

Grievance Policy / Procedure

At KAV/Izzy’s House, we are committed to addressing any concerns you may have in a timely and respectful manner. To ensure your issues are handled appropriately, please follow the process outlined below. You may also contact the Executive Director, local board (if applicable), or Ohio Recovery Housing at any time.

Steps to Address Your Concern

  1. Speak to the House Manager:
    1. If you have a concern, issue, or feel your rights have been violated, please first try to resolve the matter informally by speaking directly with the house manager.
  2. File a Written Grievance:
    1. If the issue is not resolved informally, you may file a written grievance. If you need assistance writing your grievance, please request help — Izzy’s House will provide someone to assist you.

Your Written Grievance Should Include

  1. A detailed description of the issue
  2. The date(s) when the issue occurred
  3. The names of any individuals involved

Investigation & Resolution

  • KAV will investigate, which may include interviews.
  • Within seven business days, KAV will schedule a meeting to discuss resolution.
  • You’ll receive a formal written response outlining the outcome and any steps taken.

Supplies Provided by KAV / Izzy’s

  1. Broom, dustpan, and vacuum cleaner
  2. Mop and bucket
  3. Sponges, cleaning rags, or paper towels for cleaning
  4. Appropriate cleaners for kitchen counters and bathroom sinks
  5. Toilet brush and toilet cleaner
  6. Dishwashing detergent/soap for hand washing dishes
  7. Carbon monoxide detector capable of detecting gas appliances (stove, dryer, furnace, etc.)—alarm must be loud enough to wake residents if sleeping
  8. Fire extinguisher, located in an easily noticeable spot in the kitchen
  9. Smoke detectors placed to sound alarms if there is smoke from the stove and in every bedroom
  10. Washer and dryer (if unavailable, limited, or broken, residents will have access to a nearby laundromat)

Bedding & Linens

If provided by the house, sheets, pillows, washcloths, and/or towels must be returned within 60 days or upon termination; otherwise replacement charges may apply.

Medical Cannabis Card

Izzy’s sober living does not accept or allow medical cannabis card. If a resident has cannabis in their system, it will be considered a relapse.

Medication Policy

Not Accepted Medication at KAV / Izzys

  1. Prescription narcotic medications.
  2. Morphine, stimulants, benzodiazepines/ sedative hypnotics are not permitted.
  3. Medical marijuana cards are not accepted.
  4. Over the counter or medication with alcohol in it is NOT allowed at the house.
  5. We allow suboxone16mg and under , methadone on a case by case basis (not allowed at Mitchell house), Gabapentin milligrams depending on other factors and other medications
  6. Only scheduled medication we allow is suboxone 16mg or less.

All other prescriptions are allowed only if they are prescribed to you by a doctor and are taken as prescribed.

Non-Scheduled Medications

Medications are not shared with any other resident. Residents are responsible for taking care of their own prescribed medications. Izzy’s House does not administer medications. All residents are expected to provide an accurate accounting of the medications they bring to Izzy’s House. All medications must be listed on Resident Medication Record with other pertinent information. At any time, staff deems necessary, medications may be counted to confirm the accuracy of dosages taken. Residents will not leave medications out where they are in the open or unprotected. Residents are responsible for the control of their own medications and any deviations are considered abuse. Abuse of medications will be considered a relapse and residency and disciplinary action will follow.

Residents can lock their medication in a locker if available. Izzy’s is not responsible for any stolen medications. If medications are reported stolen at the house, home managers will complete an incident report and will investigate. If residents are caught stealing, trading, sharing or giving medication it will lead to discharge.

Good Neighbor Policy

KAV/Izzy’s Sober House is committed to being good neighbors.

Resident Expectations

All residents will be informed of neighborhood guidelines including:

  • Parking in designated areas
  • Shoveling sidewalks as needed
  • Smoking only in approved locations
  • Keeping noise levels reasonable for a residential area
  • Refraining from using inappropriate language
  • Maintaining the yard and surrounding property free of trash and debris

Communication & Enforcement

  • The house manager will review these expectations with residents during the move-in process and will address and concerns as they arise.
  • Issues related to noise, smoking, or unruly behavior will be addressed promptly with residents to determine necessary actions.
  • Neighbor concerns should be directed to the house manager or the Director at 937-518-5627.
  • Residents approved by neighbors with concerns must notify the house manager or the Director immediately. If the house manager is not on-site, residents may provide the neighbor with the house manager’s contact information.
  • The house manager will document all neighbor concerns and take timely action to resolve them. Follow-up communication with neighbors will occur within an appropriate timeframe.

KAV/Izzy’s house may provide neighbors with relevant documentation from local government or information on federal fair housing laws as needed to support positive relationships.

Emergency Policy

Emergency drills will be practiced twice a year. Each drill must be documented, including the type of drill conducted and the names of all participants.

Fire

If a smoke detector sounds, or someone in the house were to notice smoke or a fire in the building, everyone must immediately exit the house.

  • Before opening any door, check if the door is hot.
  • If the door is hot, stay low and crawl to the nearest safe exit.
  • Once outside, proceed to the designated meeting spot on the sidewalk directly across from the house.
  • Remain outside and wait for instructions from the house manager, property manager, or fire department.
  • Do not re-enter the building until the fire department gives clearance.

Power Outage

In the event that the house loses power, flashlights are available for resident use. If the power outage lasts longer than 30 minutes, residents must contact the House Manager for further instructions. The House Manager will report the outage to the appropriate local utility department. The house is required to maintain an adequate supply of flashlights for emergency situations.

Water Outage

In the event that the house loses running water or if the tap water becomes unsafe to drink, bottled and filtered water will be available for residents. If the water supply is disrupted or unusable for an extended period of time, residents must contact the House Manager immediately for further instructions. The House Manager will work with the Director to develop a plan to ensure residents have continued access to safe water and necessary hygiene support.

Tornado

In the event of a tornado warning, as indicated by weather alerts or tornado sirens, all residents must immediately seek shelter in the basement. Residents are required to remain in the basement until the weather emergency has passed and it is confirmed safe to leave. If any property damage occurs as a result of the tornado, residents must contact the House Manager for further instructions and to assist with reporting and next steps.

Medical

If a resident becomes aware that another resident is experiencing a medical emergency, they must immediately call 911 and follow the instructions provided by the emergency operator. As soon as it is safe to do so—either after help has arrived or during a calm moment—the resident must contact the Housing Manager to report the incident and receive further guidance.

Overdose

Narcan (naloxone) is stored in locations throughout the house, including on top of the lockers, in the bathrooms, and in the kitchens. Instructions on how to administer Narcan are located with each kit. In the event of a suspected overdose, a resident must immediately call 911 and administer Narcan as directed. Once the situation is stable or medical professionals have arrived, the resident should contact the Housing Staff to report the incident. All residents are offered the opportunity to receive Narcan training through Project DAWN upon entry into the recovery house.

Mental Health

In the event of a mental health emergency, residents should not attempt to act as a counselor or mental health professional for another resident. Instead, they must immediately contact Housing Staff for support and guidance. If the situation is life-threatening, staff or residents will call 911 and notify the mental health agency the individual is working with to ensure continuity of care. Residents and staff may also call 988, the National Suicide & Crisis Lifeline, for immediate mental health support. If applicable, Home Managers will assist residents in completing a Safety Plan, which will be reviewed and updated monthly and as needed.

EmergencyDrills/ Safety

Staff are responsible for conducting monthly safety checks of all emergency equipment (e.g., smoke detectors, fire extinguishers, carbon monoxide detectors) and documenting these checks appropriately. Emergency evacuation drills must be conducted at least twice per year, during times when residents are likely to be present. Documentation of each drill must include the date, type of drill, and names of participants. Staff must be able to show proof that the most recent drill occurred within the last six months.

Carbon Monoxide

Staff are responsible for checking carbon monoxide detectors and other safety equipment monthly and documenting these safety checks. Everyone should immediately exit the house. Once you have exited the house via the nearest exit, remain outside the building. Everyone in the house should meet on the sidewalk directly across from the house. You will be given direction by the house manager, property manager, and/or fire department who will determine if and when it is safe to return to the house. Do not go back into the house until given instruction by the fire department to do so.

Violence or Severe Threats of Violence

In the event of violence or a severe threat of violence, staff will immediately call 911 and move to a safe location. Residents should also prioritize their safety and follow staff instructions.

Communicable Disease / Infection Control Policy

As a resident of Izzy’s House, you will be living in a community with others in recovery. People in recovery have the right to keep their personal health conditions private. Therefore, you may not be aware if someone you live with has a serious communicable disease. For this reason, it is important to always follow precautions to help prevent the transmission of communicable diseases.

KAV/Izzy’s House maintains a strict policy that standard precautions will be followed by both staff and residents when handling or disposing of hazardous materials and to ensure infection control.

  1. First-aid kits are available at each location for use in cleaning up hazardous or infectious waste. Each kit contains instructions on standard precautions and appropriate waste handling procedures.
  2. General sanitation procedures will be performed in accordance with accepted standard precaution guidelines to maintain a clean and safe environment.
  3. All staff members will receive training on the prevention and control of infections and communicable diseases, including proper use of standard precautions.
  4. Residents will be informed of expectations regarding the prevention of communicable diseases, including maintaining cleanliness, frequent hand washing, and not sharing toiletries or other personal items.
  5. Any resident who discloses to Izzy’s House that they may have a communicable disease will be connected with a healthcare provider or the public health department, as appropriate.

Izzy’s House uses designated sharps containers for biohazardous materials such as needles used for insulin or other prescribed medications. When a sharps container is full, staff will safely dispose of it by taking it to a local pharmacy or to KAV at 5563 Far Hills Drive, Dayton, Ohio.

Code of Conduct for Residents

As a resident of Izzy’s House, you agree to the following standards to maintain a safe, supportive, and respectful recovery environment:

  1. Residents will not use or possess illicit substances, recreational cannabis, or alcohol, whether on or off the property.
  2. Residents agree to treat each other with respect and provide support in one another’s recovery journeys.
  3. Residents agree not to engage in sexual or romantic relationships with other residents, staff members, or anyone with leadership responsibilities within the program.
  4. Residents agree to report any inappropriate behavior between residents, or between residents and staff/leadership, to the Director or another appropriate authority.
  5. Residents agree to adhere to curfew and be home on time. Any requests for exceptions (such as employment or other reasons) must be submitted in writing to the Home Manager for approval.
  6. Residents agree not to smoke indoors; smoking and vaping are permitted only in designated outdoor areas.
  7. Residents agree to attend all scheduled house meetings.
  8. Residents agree to follow all other house policies and procedures.
  9. Residents agree not to possess drug-related paraphernalia on the property.
  10. Residents agree not to possess firearms or any items designed as weapons on the property.
  11. Residents will actively engage in recovery planning, including setting and working toward personal recovery goals.
  12. Residents understand that immediate termination of residency may occur if there is an immediate risk to health and safety.
  13. Residents understand that violations of the Code of Conduct may result in corrective actions, such as earning points or entering motivational contracts.

Work Agreement Program

KAV understands the importance of decreasing barriers. We will offer the work agreement program under some special circumstances and when the occasion arises, it is not guaranteed.

If KAV/Izzy’s allow residents to perform work for the organization or another associated organization or business, the following has to be followed:

  • Paid work is entered into voluntarily. Residents cannot be forced to work for the organization. To be clear, a recovery housing program may require that residents get a job or seek a job, as appropriate for that particular resident in their recovery plan. However, if the resident does choose to work for an organization, it must be a free choice. The resident must have the ability to work elsewhere if they choose.
  • Work must be paid at a fair market rate. This is not just minimum wage; this is a fair market rate. For example, if a resident is performing construction work, that is generally a highly paid industry and so offering minimum wage to this person would not be acceptable. The resident must receive a fair market value for their work.
  • The work should not interfere with recovery goals. Recovery must always be allowed to come first for the resident.
  • Residents who work for an organization should not get special benefits or treatment other than the fair pay. For example, if a program allows a resident who works for them to be late for curfew due to scheduled work, the organization must also allow other residents who have outside employment to be late for curfew due to scheduled work.

Drug Screens

Sober living homes offer individuals the opportunity to make a fresh start by living with others who are also committed to achieving and maintaining sobriety. The most important condition of this living arrangement is maintaining sobriety. Therefore, some sober living homes require drug testing as a condition of residency. Drug screens may be random and/or scheduled. All new residents will be drug tested during the move-in process, with additional random tests throughout their stay. The cost of drug screens is included in the one-time application fee. Refusal to participate in drug testing may result in discharge from the program.

  1. When they complete a drug screen, they have to stay in sight of staff.
  2. Residents cannot drink more than 8 ounces of water in an hour.
  3. Residents have to drop within an hour or it is seen as a positive urine screen.
  4. If they have a positive urine screen and/or refuse , it can lead to discharge.

Visitation (If Applicable)

Residents may have visitors limited to probation and parole officers, children services representatives, peer supports, and case managers. All visitors must sign in and out upon arrival and departure. Visitors are required to remain in common areas and must follow all house rules.

Each house reserves the right to establish specific visiting dates and times that best suit the needs of the household. All visitors must be pre-approved by the Home Manager prior to their visit.

KAV reserves the right to ask any visitor to leave the property and/or prohibit their return if necessary. Visitors are strictly prohibited from possessing or being under the influence of illicit substances, recreational cannabis, or alcohol while on the premises.

Cameras

KAV/Izzy’s Sober Living prioritizes the safety of all residents and staff. To help ensure a secure environment, cameras are installed in common areas only. No cameras are located in bedrooms or bathrooms to protect privacy.

Camera access is strictly controlled and secured through sign-ins and passwords. Only current employees of KAV/Izzy’s Sober Living have access to the camera system. New employees receive individual passwords upon hire. Active employees can access camera feeds via a secure mobile application at any time. When an employee leaves the organization, their password is disabled, and all remaining employees receive updated access credentials. Camera feeds are confidential and must not be shared with anyone outside of current KAV/Izzy’s Sober Living staff.

Cameras are monitored solely for safety purposes. Staff are not permitted to engage in continuous surveillance of residents. However, if there is a report of a resident posing a threat to themselves or others, staff may view the cameras in real time to ensure safety, following the emergency policy. Recorded footage is retained for 30 days and then automatically deleted.

Camera & Camera Consent

KAV is committed to providing high-quality services within a safe and trusting environment for all residents. To support this mission, KAV/Izzy’s makes video and audio recordings of designated common areas within its facilities. These recordings help KAV/Izzy’s review practices, improve services, and maintain safety for residents, visitors, and staff.

Residents are hereby notified that KAV/Izzy’s Sober Living reserves the right to record and retain video and audio footage in common areas of all KAV/Izzy’s House locations.

By signing below, the resident knowingly and voluntarily consents to such video and audio recordings by KAV/Izzy’s House, its successors, agents, and assigns while on KAV’s property. The resident agrees to hold KAV/Izzy’s House harmless and waives any claims related in any way to these recordings. Tampering with, removing, or covering cameras is strictly prohibited and may result in immediate removal from the property.

Curfew & Overnights

KAV understands the importance of having a curfew to support residents’ recovery and safety. Curfews will be adjusted as residents progress in the house, earning increased freedoms while preparing to graduate from sober living and transition to a lower level of recovery housing. Please note that curfews and overnight passes may be revoked if residents violate house rules.

Curfew Schedule:

  1. First 3 Days
    1. Residents will be on no movement to help them get set up in treatment and learn the routine at Izzy’s Sober Living. During this time, it is strongly encouraged that residents work with their community agency and KAV/Izzy’s staff to obtain essential documents such as food stamps, social security card, birth certificate, phone, etc.
  2. First 30 Days
    1. Curfew is 10:00 PM. Residents cannot leave the house before 6:00 AM.
  3. One Month to Three Months
    1. Weekday curfew: 10:00 PM
    2. Weekend curfew: 11:00 PM
    3. Residents cannot leave the house before 6:00 AM.
    4. Residents earn two nights off per month.
  4. Fourth Month to Seven Months
    1. Weekday curfew: 10:00 PM
    2. Weekend curfew: 11:00 PM
    3. Residents cannot leave the house before 6:00 AM.
    4. Residents earn one night off per week.
  5. Seven Months to Twelve Months
    1. Weekday curfew: 10:30 PM
    2. Weekend curfew: 1:30 AM
    3. Residents cannot leave the house before 6:00 AM.
    4. Residents earn two nights off per week.
  6. Twelve Months Until Graduation
    1. Curfew will be determined collaboratively by the Home Manager and the resident.

House Meetings

House meetings are a vital part of maintaining a healthy and supportive community at KAV/Izzy’s Sober Living. These meetings encourage open communication among residents and provide a structured space to address issues, resolve conflicts, and strengthen communal living skills. House meetings also serve as an opportunity to recognize and celebrate individual and group accomplishments, and to update residents on any changes to policies and procedures. Meetings are held weekly and attendance is expected unless prior approval is given by the Home Manager. The Home Manager will lead the meetings, take notes, and ensure that a sign-in sheet is completed. All documentation from the meetings will be kept in a binder or designated file for future reference. These meetings are an important part of the recovery process, helping residents learn accountability, build stronger connections, and support one another in their journey.

House Activities

KAV/Izzy’s Sober Living provides a weekly schedule of activities that residents are encouraged to participate in. These activities may include game nights, family meals, movie nights, arts and crafts, and other community-building events. The weekly schedule will be posted in the common area of each house to ensure all residents are aware of upcoming opportunities for engagement and connection.

House Lead

We recognize the importance of structure and supportive relationships in the recovery process. To help foster this environment, each house has designated House Leads. House Leads play a vital role in building relationships with new residents and offering peer support to those who may be experiencing challenges or life stressors. They also assist in maintaining order and supporting the house when the Home Manager is not present. This is a non-paid, voluntary leadership role designed to empower residents and promote accountability within the community.

AA/NA/ Church Meetings

Residents of KAV/Izzy’s Sober Living are required to attend 12-step recovery meetings or another approved recovery support program. Residents must also provide proof of attendance at these community-based meetings. Meetings are expected to be attended in person unless prior approval for telehealth participation has been granted.

Counseling / Therapy

While living at KAV/Izzy’s Sober Living, residents are required to actively participate in community-based treatment services. This may include Intensive Outpatient Programs (IOP), Partial Hospitalization Programs (PHP), medical care, counseling, case management, and/or mental health treatment. Residents must sign a Release of Information (ROI) form for their current community provider and provide regular proof of attendance and compliance with their treatment plan. Consistent engagement in a recovery program is mandatory. KAV/Izzy’s staff will conduct routine check-ins with each resident’s treatment provider to ensure ongoing participation and progress.

Maintain Employment

At KAV/Izzy’s Sober Living, residents are encouraged to maintain a structured and purposeful routine as part of their recovery journey. Residents may be required to obtain employment to earn income, build a sense of purpose, and engage in sober social activities. If a resident is unable to find a job, they may be asked to volunteer in the community or enroll in an educational program. Residents receiving disability benefits are also encouraged to stay active by participating in meaningful activities that support their recovery and personal growth.

Community Resources

We understand the importance of connecting residents with supportive community resources. Each sober living home will maintain an up-to-date list of commonly used resources within the local community. This list will be available in the common area for residents to access, and staff will also keep a copy on hand to provide assistance as needed.

Resident Accountability Contract / 28-Day Contracts

KAV uses behavioral contracts as a motivational tool to encourage positive change and support residents in maintaining their recovery. These contracts specify which rules or policies have been violated and outline concrete steps that residents must follow to remain in housing. This process applies to any resident who breaches the Code of Conduct, providing a clear plan for remediation and fostering accountability on their path to sustained recovery.

A 28-day contract applies to residents who move in with fewer than 29 days of sobriety. Residents under this contract must agree to the following conditions:

  1. They are not to be alone in the house; there must always be another person present, such as staff, volunteers, roommates, peers, or house leads.
  2. They will have someone to check in with throughout the day.
  3. They must have assistance managing their schedule and ensure that others are aware of their plans. For example, the resident will inform the Home Manager and a peer when they are leaving or returning by sending text messages.

Exit Planning

Residents are required to provide the House Manager with a written notice at least 7 days prior to their planned move-out date. Once the move-out date is agreed upon, residents must remove all personal belongings and return their room to its original condition by 4:00 pm on the scheduled day. If an extension is necessary, it must be arranged in writing with the House Manager. A final walk-through of the room will be conducted with the House Manager before the resident leaves.

If a resident is asked to move out due to unsafe behavior—such as drug or alcohol use, violent behavior, theft—or failure to pay rent, the House Manager will escort the resident to their room to collect belongings. Arrangements for picking up any remaining items will be made with the manager. At this time, the resident must also surrender all property belonging to KAV/Izzy’s Sober Living. Any personal belongings left behind more than three days after the agreed-upon move-out date will be donated or discarded without compensation. Violation of house rules may result in immediate discharge from the program. Residents who are discharged may receive referrals to other community agencies or sober living homes in the area.

If a resident leaves and or does not return to the home, the home manager will reach out for two days and try to make contact with the resident. If the home manager cannot make contact, home manager will pack up their belongings and will hold it for three days, after three days items will be discarded or donated.

When a resident leaves, someone in management will reach out and offer the resident to complete a survey. Surveys are also kept at the houses.

No Return Policy

Residents will not be permitted to return to KAV/Izzy’s Sober Living for a minimum of one year for any of the following reasons:

  1. Engaging in physical altercations with other residents and/or staff.
  2. Spitting on residents and/or staff.
  3. Sending inappropriate sexual pictures to staff or residents.
  4. Selling drugs to residents.
  5. Committing felonies on KAV’s property.

Resident Agreement & Acknowledgment

I have read and agree to follow the rules of KAV/Izzy’s. I understand that failure to comply with these rules may result in termination of my lease. I also understand that rent is due every Friday by 2:00 PM.

Carpool with KAV/Izzy Residents Release

I, (resident name), understand that due to lack of my own personal transportation, I may request transportation from time to time from a resident of KAV Health Group, LLC. I hereby indemnify KAV Health Group, LLC. and their staff from all damage or injury caused to me or others when I willingly accept transportation to or from any location or event, whether KAV Health Group, LLC related or not.

KAV / Izzy’s Housing – Resident Rules

  1. I will not possess any drugs, alcohol, or mind-altering substances
    1. This includes ‘non-alcoholic’ beer or wine.
    2. Staff must be notified in writing of any prescription medications. Mouthwash or medications with alcohol are not allowed. Only over-the-counter medication that is on the “Safe List” may be used.
    3. Residents must store all medications in a safe spot and will be shared with the housing manager.
    4. Sharing of medications with other residents is not allowed.
    5. Alcohol or drug use is grounds for immediate termination of the right to remain in the KAV Health Group, LLC program, at which time a recommendation for a higher level of care could be made.
    6. No drug-related paraphernalia of any kind is allowed
    7. Some medications are NOT allowed regardless of the reason. Please contact the housing manager for a list of these medications.
  2. I will agree to take random drug/alcohol tests when requested by KAV Health Group, LLC staff.. Refusal or failure to take the test immediately, or a positive test, will result in immediate dismissal from KAV/Izzy’s House, with a recommendation for a higher level of care.
  3. I acknowledge my responsibility to contribute to a safe and supportive environment. This includes openly communicating with staff about any concerns or suspicions regarding chemical use, rule violations, or destructive behaviors among community members. I recognize that failure to report such issues may compromise the well-being of the community and may result in my discharge from the program.
  4. I agree to attend and be on time for all KAV/Izzy’s House meetings.
  5. I agree to attend two 12 step meetings weekly during my stay at KAV/Izzy’s House. I agree to comply with the documentation of such and turn it in every Monday.
  6. I must be medically appropriate for KAV/Izzy’s House. If I am not, this could result in dismissal from the sober living program.
  7. Recovery will be my #1 priority above all else (work, relationships, etc.)
  8. Residents are expected to gain employment during their stay at KAV/Izzy’s House. Gaining employment is permitted, once outside recovery treatment and/or medical recovery is compliant and compliant with house rules and expectations, as soon as Day #4. Work schedules must be submitted to the housing manager to ensure Programming Requirements are not expected to be compromised or modified for work privileges.
  9. Residents are expected to submit three employment applications weekly.
  10. I agree to abide by curfew.
  11. I agree not to have any sexual relationship with anyone living in my residence while living at the house.
  12. I understand that violence, or threats of violence, emotional or physical will not be tolerated and are grounds for immediate dismissal and possible legal action.
  13. I will not bring weapons into the community - including stored in a vehicle.
  14. I will not participate in gambling on the premises.
  15. I will be respectful of cell phone policies:
    1. No phone use during house meetings
    2. Use of headphones while in public places.
    3. Be respectful of your peers after hours.
  16. KAV Health Group, LLC staff reserves the right to search personal property at any time while I am a resident; with or without the resident present.
  17. Guests are permitted per housing location rules, reach out to home manager for details
  18. I will, at all times, respect the other members of the house in regard to noise, eating, cleaning personal space, personal possessions and common living areas.
  19. As a sober living resident, I acknowledge that cleanliness and organization are crucial. I commit to contributing to household chores and responsibilities, participating in community-established cleaning schedules, and maintaining a tidy personal space, including daily bed-making and trash removal. This shared responsibility ensures a smooth-running and respectful community environment.
  20. No sleeping on the couches. This is a common area to be utilized by all household members.
  21. No storage space is available in housing, beyond what can fit in one’s bedroom.
  22. I will be fully dressed in the common areas at all times.
  23. KAV Health Group, LLC is NOT responsible for losses inside or outside the house, or theft of personal property, including money, jewelry, clothing, etc. Stealing will result in immediate termination and possible legal action. We encourage residents to get renters insurance.
  24. When departing KAV/Izzy’s House, residents are responsible for leaving their space clean and tidy, including vacuuming, and returning all items provided by the organization. Personal belongings must be collected within 48 business hours of departure, and arrangements should be made with the House Manager for pickup. Please note that KAV Health Group, LLC/Izzy’s House cannot be held responsible for items left behind after this timeframe. Any unclaimed belongings will be donated to charity.
  25. At no time am I allowed in another resident’s room, for any reason! All conversations, activities, visiting, etc. is to be done in the common areas. There are no exceptions. If I do not live in a particular room, I do not go there.
  26. I understand loaning personal items such as money, clothing and/or food is at my own risk and staff will not be responsible or liable for any personal items.
  27. I understand that smoking is only allowed on the outside in designated areas only. Residents who smoke must always be kept clean. Butts need to be picked up and disposed of appropriately. No tobacco products may be used inside the house, including electronic cigarettes and vapes. NOTE: Smoking in the house will result in dismissal from the house. No vaping in the house.
  28. No halogen lamps, candles, or incense are permitted to be burned in the sober living house. NO outside fires are allowed including burning leaves or trash. This will result in immediate removal from the house.
  29. I understand that pornographic materials are not allowed in the residences this includes but is not limited to Internet pornography sites, magazines, video materials, etc.
  30. No pets.
  31. You are not allowed to remove or move any furniture or fixtures from the house or from room to room. Do not bring any of your furniture or it becomes KAV Health Group, LLC’s property. All items in the house or hanging on the walls are KAV Health Group, LLC property. You may bring family pictures or recovery items only. These items are allowed only in free standing frames. No wall hangings will be allowed. We are not responsible for storing personal items and space is limited. Notify the House Manager if you are donating something to your house with the understanding that the item stays after you move out..
  32. KAV/Izzy’s residents are required to stay compliant with clinical services, not being compliant could result in dismissal of KAV/Izzy’s housing or being referred out to an inpatient if using drugs or alcohol.
  33. Izzy's House is a safe environment for residents and staff. Disrespect and/or bullying WILL NOT be tolerated and are grounds for immediate removal/discharge!
  34. I know staying at KAV/Izzy’s I have to be in treatment.
